To apply, click on the statement below that describes you (if more than one statement applies, choose one). Print out and complete the corresponding application, making sure to enclose all necessary documentation before mailing. If you need assistance, call 1-866-RELAY-MT (voice/TTY).

I am a Native American. (You must be a member of a federally recognized tribe to receive a phone through this program. Click here for a list of these tribes.)
